Verification requests must be made in writing and include the following:

• Certificant's full name
• Certificant's home address
• Certificant's HRCI ID number
• Certificant's designation (e.g.. PHR, SPHR, or GPHR) and the year in which they became certified
• Written verification from the certificant to allow designation information to be released to the requestor.
